These Terms of Use govern access to and use of the Camberwell Institute website and any services scheduled or requested through the website. By accessing this website or scheduling services through the website, you agree to these Terms of Use.

1. Informational Purpose of Website

Information provided on this website is for general informational purposes only. Use of this website does not establish a physician-patient relationship unless and until an appointment has been scheduled and accepted by the practice.

2. Eligibility

Services are intended for individuals 12 years of age or older. Individuals under the age of 18 must have the involvement of a parent or legal guardian as required by applicable law.

3. Nature of Services

Camberwell Institute provides psychiatric and mental health services, including diagnostic evaluations, autism assessments, ADHD assessments, and related clinical services.

Services are delivered primarily through telehealth.

In-person appointments may occasionally be available depending on physician availability and location. Most patients are seen virtually. Initial encounters are typically conducted through telehealth. Hybrid or in-person follow-up appointments may be offered only in limited circumstances and at the physician’s discretion.

4. Evaluation vs. Diagnosis

When scheduling an evaluation or assessment, patients are purchasing a professional evaluation service, not a guaranteed diagnosis or specific clinical determination.

Clinical conclusions, diagnoses, and recommendations are determined solely by the evaluating physician based on professional medical judgment, clinical standards, and the information available at the time of the evaluation.

5. Physician Discretion

The physician reserves the right to determine the clinical appropriateness of requested services. If a requested service is determined to be inappropriate, outside the scope of the practice, or clinically unsuitable, the practice may decline to provide that service.

Clinical procedures, documentation requirements, and information collected during care may vary depending on state licensing requirements, regulatory obligations, and professional standards.

6. Autism and ADHD Evaluations

Autism and ADHD assessments may involve clinical interviews, structured assessment tools, specialized testing, and physician review of clinical history.

If the physician determines that it is clinically appropriate, a written diagnostic report may be prepared following the evaluation. Such reports may be used for purposes including school or workplace accommodations.

Documentation and written reports are not provided immediately following the evaluation. Preparation of a physician diagnostic report may take up to eight (8) weeks depending on clinical workload and documentation requirements.

In some cases, an expedited documentation service and corresponding fee may be available. Expedited documentation is not guaranteed and may be offered only at the physician’s discretion depending on workload and availability.

7. Prior Evaluations and External Screenings

Patients may provide prior evaluations, screening tools, or documentation completed by other professionals, including social workers, therapists, psychologists, pediatricians, family physicians, internal medicine physicians, general practitioners, or school psychologists.

These records may be reviewed as part of the clinical history; however, a full evaluation performed by the physician through this practice is required in order to obtain a physician-level diagnostic report through this practice.

8. Patient Responsibilities

Patients are responsible for providing accurate and complete information prior to and during appointments, including but not limited to medical history, psychiatric history, family history, current medications, treating physicians or clinicians, physical location at the time of the telehealth appointment, and completed intake forms and questionnaires.

Failure to provide required information may prevent the physician from proceeding with care or prescribing medication.

Patients are responsible for attending scheduled appointments on time and being prepared to participate in the scheduled service.

9. Payment Policy

Payment for services is required in advance at the time of scheduling.

Fees secure the physician’s time and scheduled appointment slot. Physician time is reserved in advance, and the practice compensates physicians for scheduled clinical services regardless of whether a patient completes the appointment.


10. No Refund Policy

All payments for scheduled services are non-refundable.

By scheduling and submitting payment, you acknowledge and agree that fees paid for scheduled services are not refundable.

11. Recording and Confidentiality

Recording of any clinical interaction with the physician or other care providers is strictly prohibited.

This includes audio recording, video recording, screen recording, or any other method of capturing the interaction during a telehealth session or clinical encounter.

Unauthorized recording, distribution, or sharing of clinical interactions may violate privacy laws and the rights of the physician and practice. Any such recording or distribution may be subject to legal action, including claims for damages and financial restitution to the fullest extent permitted by law.

Patients must also disclose the identity of any individual present or listening during a telehealth appointment. The physician may decline to proceed with care if unauthorized individuals are present or if disclosure is not provided.

12. Controlled Substances

This practice does not provide prescribing of controlled substances.

13. Emergency and Crisis Services

This practice does not provide emergency services, crisis intervention, or urgent psychiatric care.

If you are experiencing a psychiatric or medical emergency, you should call 911 or go to the nearest emergency department.


14. No 24-Hour Coverage

The practice does not provide 24-hour clinical support or on-call services. Communications submitted through the website, patient portal, or email are not monitored continuously.

15. Appointment Availability

Appointments are scheduled subject to physician availability. While same-day appointments may occasionally be available, same-day or urgent appointments are not guaranteed.

16. Telehealth Acknowledgment

By scheduling services through the website, you acknowledge that care may be delivered through telehealth technologies. Telehealth services rely on internet and communication technologies and may be subject to technical limitations.

17. Website Use

Users agree not to use the website for unlawful purposes, attempt unauthorized access to website systems, or interfere with website functionality or security.

18. Intellectual Property

All website content, including text, graphics, branding, and design elements, is the property of Camberwell Institute unless otherwise stated and may not be reproduced or used without permission.
